Enrollment and Medical Forms
What do you need to do to join Cub Scouting? You need to do a few things: First, fill out a Youth Application (also available as a Youth Application En Español). Note that this is normally a carbon-copy form, if you fill out this PDF version, you will have to enter the same information on each of the several pages. Carbon-copy forms are available at Pack functions. Fill out parts A and B of the standardized 2011 Medical Form (replaces all forms prior to January 1, 2011). Part C requires a doctor's visit, and is required for events longer than 72 hours, which applies mostly to Boy Scout programs. Part D is not relevant for Cub Scouts (it's for Boy Scouts on high-adventure programs), and is not included in this file. Fill out a Cub Scout Information Sheet. Write a check for the Cub Scout registration fee, payable to "Pack 3371". Fees are $15/year, plus $2.00 for insurance. Registration is pro-rated, so the amount you need to pay is $1.25/month, plus two dollars. EXAMPLE: If you sign up on September 1st, you pay registration for September-December. That's 4 months plus two dollars, so: If you want to receive "Boy's Life" magazine, the cost is $1.00/month; add this to your registration fees. In the above example, Boy's Life for September - December would be an additional $4.00. Bring all of these to a Pack or Den function (see our Events Calendar), and give them to the leader in charge. Most registration is done at our Round-Up meetings in August, at the start of the school year, but boys are welcome to join at any time. If you are transferring from another Pack (for example, if you just moved to Lafayette), you still fill out these same forms. HOWEVER, on the Youth Application, mark that you are a transfer and provide information about your previous unit, so that we can get your records. Transfers don't pay full registration costs; the fee for boys transferring is $1.00, at any time of year. |
